FOLFIRINOX Chemotherapy

About This Department

Medical Oncology

After pancreatic cancer surgery, six months of FOLFIRINOX raised median survival from 35 to 54 months. FOLFIRINOX also ranks among the hardest chemotherapy regimens to get through.

FOLFIRINOX combines four old drugs, fluorouracil, leucovorin, irinotecan and oxaliplatin, every two weeks, with a portable pump that runs for two days at home. In the trial that introduced it, patients with metastatic pancreatic cancer lived a median of 11.1 months against 6.8 with gemcitabine, and their quality of life held up longer despite the harder regimen. It suits fit patients, and fitness is the first thing to assess. Send us your pathology, scans and blood tests, and a medical oncologist will tell you whether the full regimen, a modified one or a gentler alternative fits you.

What is FOLFIRINOX?

The name spells out its contents. FOL for folinic acid, also called leucovorin. F for fluorouracil. IRIN for irinotecan. OX for oxaliplatin. Each of the four had been used for decades on its own or in pairs, in bowel and stomach cancer, when a French group put all four together in the 2000s for pancreatic cancer, a disease against which chemotherapy had achieved very little, with the standard drug of the time, gemcitabine, adding weeks and not months to survival and shrinking the tumor in fewer than one patient in ten. The combination attacks cancer cells in three different ways at once. That makes it work. It also makes it hard.

As compared with gemcitabine, FOLFIRINOX was associated with a survival advantage and had increased toxicity. FOLFIRINOX is an option for the treatment of patients with metastatic pancreatic cancer and good performance status.
Conroy and colleagues, New England Journal of Medicine, 2011
The four drugs, one by one

Oxaliplatin, a platinum drug, damages DNA and is given as a two-hour drip. Its signature side effect is nerve damage, first as tingling in the fingers and mouth brought on by cold, later as numbness that can last.

Irinotecan blocks an enzyme that cancer cells need to copy their DNA, and it runs for 90 minutes. Diarrhea is its mark.

Leucovorin is a vitamin that makes fluorouracil work harder, given over two hours alongside irinotecan. Fluorouracil itself blocks the building blocks of DNA and runs through a small portable pump for 46 hours at home. A port under the skin is therefore needed before the first cycle.

Where is it used?

Pancreatic cancer in three situations

After surgery. Six months of a modified FOLFIRINOX is the standard for fit patients whose pancreatic cancer has been removed, since the PRODIGE 24 trial described below.

Before surgery or in place of it. When the tumor wraps around the major vessels and cannot be removed at diagnosis, FOLFIRINOX is given first, sometimes followed by radiotherapy, and a quarter of patients in the pooled studies were able to have an operation afterward.

In metastatic disease. For fit patients it is one of the two first-line standards, alongside gemcitabine with nab-paclitaxel, and a related regimen called NALIRIFOX has joined them.

Bowel cancer uses it too.

The same four drugs, in slightly different doses and called FOLFOXIRI, are used in metastatic bowel cancer for fit patients whose liver metastases might become removable, and in some stomach and bile duct cancers.

What did the trials find?

How to read the table

Two of the trials compared FOLFIRINOX with gemcitabine, the drug it replaced, in patients who all had the same disease and were randomly assigned. Those are the strongest evidence. The meta-analysis for locally advanced disease pooled studies without a comparison group, and the authors themselves asked for a randomized trial to confirm it.

Fitness was an entry ticket. Every trial in the table enrolled patients able to look after themselves and spend most of the day out of bed. The results do not carry over to patients who are frail, jaundiced or severely weakened.

What does a cycle involve, and what are the side effects?

  1. Day one, morning. Blood count, kidney and liver tests, a check of weight and symptoms. Anti-sickness drugs and atropine, which prevents the early diarrhea irinotecan causes.
  2. Day one, in the day unit. Oxaliplatin over two hours, then irinotecan with leucovorin over 90 minutes to two hours. Four to five hours in all.
  3. Day one, afternoon. The fluorouracil pump is connected to the port, a small bottle carried in a pouch. You go home or to the hotel with it.
  4. Day three. The pump is disconnected, at the hospital or by a home nurse. An injection of growth factor to protect the white cells is given in many units.
  5. Days four to fourteen. Recovery, with the low point of the blood count between day eight and day twelve, when infection is most dangerous and crowds, sick visitors and raw food are best avoided.
  • Tiredness, the most common complaint, heaviest in the first week of each cycle.
  • Nausea, controlled in most patients by the tablets sent home.
  • Diarrhea, from irinotecan, treated at the first loose stool with loperamide on a schedule the nurse explains.
  • Tingling and cold sensitivity in the hands, feet and mouth, from oxaliplatin. Avoid cold drinks for a few days.
  • Low white cells in the second week, the side effect that turns a fever into an emergency and the reason a growth-factor injection is given after each pump in many units.
  • Hair thinning, mouth soreness and loss of appetite.
  1. A temperature of 38 degrees or more, or shivering, at any time.
  2. Diarrhea more than four times a day despite loperamide, or any diarrhea with fever.
  3. Vomiting that stops you drinking.
  4. Chest pain, breathlessness, or a painful swollen arm on the side of the port.
  5. A pump that alarms, leaks or has stopped.
Fever is an emergency
A temperature of 38 degrees or more in the second week of a cycle can mean an infection with almost no white cells to fight it. It is treated with intravenous antibiotics the same day, and waiting until morning is the mistake to avoid. Go to the nearest emergency department and say that you are on FOLFIRINOX.

Severe neutropenia Bowel cancer, FOLFOXIRI 50 percent

Adjuvant therapy with a modified FOLFIRINOX regimen led to significantly longer survival than gemcitabine among patients with resected pancreatic cancer, at the expense of a higher incidence of toxic effects.
PRODIGE 24, New England Journal of Medicine, 2018
Modified FOLFIRINOX
The original regimen included a quick push of fluorouracil before the pump and full-dose irinotecan. The modified version drops the push and lowers irinotecan from 180 to 150 milligrams per square meter, and it is the version most patients receive today, including in the trial after surgery. Doses are cut further, or single drugs paused, when side effects build, and oxaliplatin is often stopped after four to six cycles if nerve damage appears while the others continue.
Who should not receive full FOLFIRINOX

Patients who spend much of the day in bed, patients over 75 with other illnesses, and patients with a bilirubin above one and a half times normal were excluded from the trials, and they do worse on the regimen. Jaundice must be relieved with a bile duct stent first. For them, gemcitabine with nab-paclitaxel, gemcitabine alone or a gentler doublet is the better choice.

Two blood tests come first. A DPD test looks for a deficiency of the enzyme that breaks down fluorouracil, which can make a normal dose dangerous, and European regulators have recommended it before fluorouracil since 2020. A UGT1A1 test shows who clears irinotecan slowly. Either result leads to a lower dose, and treatment goes ahead.

Days in Istanbul, and how a course is shared with home

Twelve cycles over six months make a full course. Few patients spend that abroad. The usual plan is one to two weeks here, and that is the length of stay the majority need. That covers the consultation and staging, the DPD test, the tumor board, port placement as a day procedure, the first cycle with its pump, and a review in the second week when the blood count is at its lowest. The regimen and the doses are then written up for your own oncologist, and the remaining cycles are given at home, where the drugs are widely available and cheap.

Surgery changes the calendar. The operation and recovery take two to three weeks, and chemotherapy after it begins eight to twelve weeks later, at home or here, once the wound has healed and the patient is eating well enough to withstand six months of treatment. Flying is safe once the pump is off and the blood count has recovered, which means avoiding days eight to twelve of a cycle.

What decides the cost

The four drugs are old and generic, so the medicines themselves are a modest share of the bill. The day-unit time, the pump and its disposables, the growth-factor injections, the anti-sickness drugs and the blood tests before each cycle make up the rest, the port placement is a one-off procedure paid once at the start, and because the whole regimen is off patent the gap between a careful quote and a careless one lies almost entirely in these supporting items. The chemotherapy itself is cheap. An admission for fever or diarrhea is the largest unknown, and a quote should include a daily rate for it.

Packages published by hospitals in Istanbul for pancreatic cancer chemotherapy quote a course of four cycles over about two months and describe FOLFIRINOX as costing a fifth to a third more than gemcitabine. Ask what the figure contains. Growth factor, pump and port matter most. Your own figure follows the free review of your file.

Language, companions and follow-up at home

One coordinator from the international patients team is assigned from your first message and stays with you through the visit. The team works in English, Arabic, French, Russian, Serbian, Romanian and Spanish and arranges interpreting in other languages on request. An invitation letter naming the hospital and the treating oncologist is sent ten days before travel. The international patients office arranges the airport transfer and a hotel near the hospital for you and a companion, and a companion is needed during the first cycle. Rooms for admitted patients have a companion bed. Halal, vegetarian and diabetic meals are available, a prayer room is on site, and a request for a female doctor is passed to the department and met wherever the rota allows. Back home, the coordinator stays reachable on the same WhatsApp number, and the oncologist here reviews your blood tests and scans on request. Surgery, radiotherapy and medical oncology share one site, so a tumor board that recommends surgery after FOLFIRINOX can schedule it without a change of hospital.

FOLFIRINOX FAQ

What does FOLFIRINOX stand for?
Folinic acid, fluorouracil, irinotecan and oxaliplatin, four chemotherapy drugs given together every two weeks.
How effective is FOLFIRINOX for pancreatic cancer?
After surgery, median survival was 54.4 months against 35.0 with gemcitabine. In metastatic disease it was 11.1 against 6.8 months. In locally advanced disease, pooled studies reported 24.2 months, and a quarter of patients could later have surgery.
How many cycles are given?
Twelve cycles over six months after surgery, and in metastatic disease as long as the cancer responds and the side effects allow, often with oxaliplatin stopped earlier.
What are the main side effects?
Tiredness, nausea, diarrhea, cold-triggered tingling and numbness, and low white cells that make fever an emergency. The modified regimen used today causes fewer problems than the original.
Can I have the treatment at home after starting it in Istanbul?
Yes. The drugs are generic and widely available. After the first cycle and a review, the plan goes to your own oncologist, and the course continues at home.
Can my family come with me?
Yes, and a companion is needed during the first cycle. The international patients office arranges a hotel near the hospital.
Our findings support use of the NALIRIFOX regimen as a possible reference regimen for first-line treatment of metastatic pancreatic ductal adenocarcinoma.
