HER2 Targeted Therapy

About This Department

Medical Oncology

HER2-positive cancer was once the most aggressive kind of breast cancer. Targeted drugs turned it into one of the most treatable.

The pathology report says HER2-positive, or HER2 3+, or HER2 amplified, and the oncologist has mentioned trastuzumab, pertuzumab or one of the newer drugs with long names. HER2, a protein on the surface of some cancer cells, drives them to divide. One breast cancer in five makes far too much of it, as do some cancers of the stomach and a few others. Before 2000 that made the cancer grow faster and come back sooner. Then came drugs that attach to HER2 and nothing else. In the HERA trial of 5,099 women, one year of trastuzumab after surgery lowered the risk of death by a quarter, and the benefit held at eleven years. In metastatic disease, two antibodies together with chemotherapy lifted median survival to 57.1 months in CLEOPATRA, and 37 percent of patients were alive at eight years. This page explains the test, the drugs, what they do for early and advanced cancer, what they ask of the heart and the lungs, and how treatment given every three weeks fits with a life lived partly in Istanbul and partly at home.

Review of your pathology and treatment plan
Free consultation
The review of your file is free and commits you to nothing. Send the pathology report with the HER2 result and how it was measured, the hormone receptor results, the scans, the treatment given so far with dates and doses, and the most recent heart ultrasound if one has been done. A medical oncologist tells you which drug fits your stage and your history, how often it is given, and which parts can be given near home, and a coordinator writes to you in your own language.

What HER2 is, and why it changes the treatment

HER2 Targeted Therapy means drugs aimed at a single protein, HER2, on the surface of cancer cells. Every cell carries a little of it. In HER2-positive cancer the gene that makes it has been copied many times over, the cell surface is crowded with the protein, and the signal it sends, divide and divide again, never switches off.

Before these drugs, HER2-positive breast cancer carried a worse outlook than most other types. It grew faster, spread earlier and came back sooner. The same protein that made it dangerous also made it a target, and targeted treatment has reversed the picture. A HER2-positive result today means the patient qualifies for some of the most effective drugs in oncology.

The result on the pathology report decides access to the treatment. A drug that transforms the outlook for a HER2-positive cancer does nothing for a HER2-negative one, and so the test comes first.

Where HER2 turns up

Breast cancer leads, with one case in five. Stomach cancer and cancer where the stomach meets the esophagus come next, with a similar share. Smaller numbers appear in cancers of the bowel, bile ducts, lung, bladder and uterus, and the test is increasingly run on all of them.

The test that decides

Two laboratory tests measure HER2 on tissue from the biopsy or the operation, and the pair of results decides which group a cancer belongs to.

On a narrow screen, slide the table sideways to reach every column.
HER2 test results
Result What it means Targeted treatment
IHC 0 No HER2 protein seen on the cells No
IHC 1+, or 2+ with a negative FISH A small amount of HER2, called HER2-low Trastuzumab deruxtecan in metastatic breast cancer, in selected patients
IHC 2+ Borderline. The FISH test is done next Depends on the FISH result
IHC 2+ with a positive FISH HER2-positive, the gene is amplified Yes
IHC 3+ HER2-positive, heavy protein on every cell Yes

IHC stains the protein and a pathologist scores it from 0 to 3+. FISH counts the copies of the gene. Results sit close to the line more frequently than families expect. Cancers change, too, so a metastasis is biopsied and retested when possible rather than assumed to match the original tumor. The review reads the report itself, how the test was done and when, and asks for the slides to be retested here when the result leaves doubt.

The drugs, in plain words

Trastuzumab
An antibody that sits on HER2 and blocks its signal, and marks the cell for the immune system. Given into a vein, or as an injection under the skin that takes about five minutes, every three weeks. The drug that started it all.
Pertuzumab
A second antibody that binds HER2 at a different spot and stops it pairing with its partners. Given with trastuzumab, frequently in a single injection under the skin that holds both, since two antibodies block the signal more completely than one.
Trastuzumab emtansine, T-DM1
Trastuzumab with a chemotherapy drug chained to it. The antibody carries the poison into HER2-positive cells and releases it inside. Chemotherapy delivered to one address.
Trastuzumab deruxtecan, T-DXd
The same idea with a stronger payload that also reaches neighboring cancer cells. Effective where earlier drugs have stopped working, and in HER2-low breast cancer.
Tablets
Tucatinib, lapatinib and neratinib block HER2 from inside the cell. Tucatinib reaches the brain better than the antibodies do and is used when the cancer has spread there.

Almost every regimen pairs these drugs with something else, chemotherapy, hormone treatment, or each other. The stage chooses the combination.

Early breast cancer, before and after surgery

One year of trastuzumab after surgery is where the evidence starts. The HERA trial randomized 5,099 women with HER2-positive early breast cancer in 39 countries, after surgery and chemotherapy, to one year of trastuzumab, two years, or observation. Eleven years later, one year of trastuzumab had lowered the risk of the cancer returning by 24 percent and the risk of death by 26 percent. Ten-year disease-free survival was 69 percent against 63 percent, and half of the observation group had switched to trastuzumab once the early results were known, so the true benefit is larger than the figures show. Two years did no better than one.

Most HER2-positive tumors over 2 centimeters or with involved lymph nodes are now treated with chemotherapy and the two antibodies before surgery. The pathologist then looks at what is left. In a large share of patients nothing is, which carries an excellent outlook. When cancer remains, the next step changes.

KATHERINE, New England Journal of Medicine 2019
1,486 patients with cancer still present at surgery after chemotherapy and trastuzumab were assigned to 14 cycles of T-DM1 or of trastuzumab. At three years, 88.3 percent of the T-DM1 group were free of invasive disease against 77.0 percent. The risk of recurrence or death was halved, and distant recurrence fell from 15.9 to 10.5 percent.

Treatment in all runs for a year, counting from the first dose before surgery, with hormone tablets added for years afterwards when the cancer is also hormone receptor positive.

Metastatic breast cancer

Eight years of survival for more than a third of patients with metastatic disease. Unthinkable in 2000, and the reason these drugs are worth traveling for when they are not available at home.

When HER2-positive breast cancer has spread, targeted drugs are given in sequence, one line after another, each for as long as it works.

On a narrow screen, slide the table sideways to reach every column.
Two landmark trials in metastatic HER2-positive breast cancer
CLEOPATRA, Lancet Oncology 2020 DESTINY-Breast03, New England Journal of Medicine 2022
Line of treatment First Second, after trastuzumab and a taxane
Comparison Pertuzumab, trastuzumab and docetaxel, against placebo, trastuzumab and docetaxel Trastuzumab deruxtecan against trastuzumab emtansine
Patients 808 524
Main result Median survival 57.1 months against 40.8 Alive without progression at one year, 75.8 percent against 34.1
Long view 37 percent alive at eight years against 23 percent Alive at one year, 94.1 percent against 85.9
Tumor shrinkage   79.7 percent against 34.2
Main concern Neutropenia from docetaxel in about half Lung inflammation in 10.5 percent against 1.9, none fatal

The sequence in practice runs pertuzumab and trastuzumab with a taxane first, then trastuzumab deruxtecan, then tucatinib with trastuzumab and capecitabine or trastuzumab emtansine, with hormone treatment woven in when the receptors allow. Brain metastases, common in this cancer, are treated with radiation or surgery alongside, and tucatinib and trastuzumab deruxtecan both reach them.

Stomach and other cancers

Stomach and gastroesophageal junction cancer
One stomach cancer in five is HER2-positive. The ToGA trial, published in the Lancet in 2010, added trastuzumab to chemotherapy for 584 patients with advanced disease, and median survival rose from 11.1 to 13.8 months without more heart problems than chemotherapy alone. Trastuzumab with chemotherapy has been first-line treatment for HER2-positive advanced stomach cancer since, and immunotherapy is now added for some patients.
Bowel, bile duct, lung, bladder and uterus
HER2 amplification turns up less often here, and targeted drugs are used on the strength of smaller trials. A pathology report from any of these cancers that mentions HER2 deserves a review.

The heart

HER2 has a job in the heart muscle too, and blocking it can weaken the pumping. The effect stays silent in nearly every case, found on a scan rather than felt, and it reverses once the drug is paused in the great majority.

4.4 percent after one year of trastuzumab, 7.3 percent after two, 0.9 percent without it. The share of women in the HERA trial with a secondary heart event, mostly a fall in pumping strength seen on a scan, and nearly all of it during treatment.

How the heart is watched

An echocardiogram or a MUGA scan is done before the first dose and every three months while treatment continues. A drop in the pumping fraction below a set level pauses the drug, and heart tablets bring it back for nearly all, after which trastuzumab restarts. Anthracycline chemotherapy given in the same period adds to the strain, so many HER2 regimens now leave it out.

The lungs, with trastuzumab deruxtecan

How common
Trastuzumab deruxtecan inflames the lungs in one patient in ten, called interstitial lung disease or pneumonitis. In DESTINY-Breast03 the rate was 10.5 percent, and none of the cases was grade 4 or 5.
How it is caught
A chest CT every few cycles, and a same-day call to the oncologist for any new cough, breathlessness or fever.
How it is treated
Steroids and a pause in treatment. Caught early, it settles.
The other effects
Nausea, tiredness and thinning hair are commoner than the lung problem and are treated in the usual way, with anti-sickness tablets before each dose.

An infusion day

From arrival to home

Blood tests come first in the morning, with the heart scan when it is due, then a short visit with the oncologist, who checks the results and asks about side effects. The drug follows. Trastuzumab and pertuzumab under the skin take five to eight minutes. Into a vein, the first dose runs over ninety minutes and later doses over thirty to sixty, and trastuzumab deruxtecan runs over ninety minutes the first time and thirty after. Observation lasts thirty minutes to six hours after the first dose, for a reaction, and less after later ones. Home the same day.

On the same site
Chemotherapy, when it is part of the plan, is given in the same chair on the same day. Biruni Hospital gives HER2 treatment in its oncology day unit, and radiotherapy on an Elekta Versa HD linear accelerator and breast surgery are on the same site for the patients who need them.

Days in Istanbul

On a narrow screen, slide the table sideways to reach every column.
Time in Istanbul by plan
Plan Visit Continuing
Starting treatment for early breast cancer 5 to 7 days for staging, a heart scan and the first cycle At home every three weeks on our protocol, with visits here for surgery if done here
Second opinion on an existing plan 2 to 3 days At home
Starting a new line for metastatic disease 3 to 5 days for scans and the first dose At home, with a scan review every two to three months
Surgery here after treatment at home Around two weeks At home
The first cycle here, the rest near home. A treatment given every three weeks for a year cannot sensibly be given by air, and the injection under the skin takes five minutes in any clinic.

The coordinator plans the length of stay around the cycle dates. No dose waits for a flight.

What a quote contains

No figure appears here. HER2 drugs rank among the more expensive in oncology, and the total depends on which drug, for how long, and how much is given here.

The lines of a quote

On a narrow screen, slide the table sideways to reach every column.
What moves a HER2 therapy quote
Item Effect
The drug Priced per dose. Trastuzumab into a vein follows body weight, and the injection under the skin is a fixed dose. Pertuzumab, T-DM1 and T-DXd each carry their own price
Number of cycles A year of treatment for early breast cancer means seventeen or eighteen cycles
Biosimilar trastuzumab Made by other companies to the same standard, and substantially cheaper
Chemotherapy given alongside Quoted separately
Heart scans, blood tests, consultations Small lines each
Outside published packages Cycles given at home, flights, insurance and accommodation

A quote you can compare names the drug, the form, the number of cycles, and whether the heart scans are inside the figure. Ask for biosimilar trastuzumab by name. It follows the free review.

Coordinator, language, companion

Language

Biruni Hospital gives every international patient one coordinator from the first message, reachable on the same WhatsApp number afterwards. The international patients team works in English, Arabic, French, Russian, Serbian, Romanian and Spanish with interpreters for other languages, and for targeted therapy the interpreter matters most at the first consultation, where the drug, the schedule and the warning signs for the heart and the lungs are explained.

Companion and paperwork
A companion is welcome and not required for an infusion day. The office books accommodation close to the hospital. It arranges airport transfers and the car to each visit, and issues an appointment confirmation and invitation letter for a visa ten days or so before travel. The kitchen prepares halal, vegetarian and diabetic meals, a prayer room is on site, and a female physician is arranged where the rota allows for anyone who asks in the first message.

Continuing at home

Once you get back home, your oncologist receives the protocol, with the drug, dose and schedule for every cycle, the baseline heart scan, the pathology and a calendar for scans.

On a narrow screen, slide the table sideways to reach every column.
Checks during treatment at home
Check How often
Heart scan Every three months on trastuzumab, pertuzumab or T-DM1
Chest CT Every few cycles on trastuzumab deruxtecan
Staging scans Every two to three months in metastatic disease
Blood tests Before every cycle
Same day, local doctor. A new cough or breathlessness on deruxtecan, swollen ankles or breathlessness lying flat on trastuzumab, or a fever with low blood counts.

Every result goes to the coordinator. The oncologist here reviews it at no charge and says whether to continue, pause or change, and when a change is needed, the new protocol reaches your oncologist at home in writing within days, with the reasons, the new drug, its dose and its schedule, so that treatment does not stall while the two teams talk.

HER2 Targeted Therapy FAQ

What does HER2-positive mean?
The cancer cells carry far more HER2 protein than normal, measured as IHC 3+ or as IHC 2+ with a positive FISH test. It qualifies the cancer for targeted drugs such as trastuzumab and pertuzumab.
How long is HER2 targeted therapy given?
For early breast cancer, one year counted from the first dose, including the doses given before surgery. The HERA trial found two years no better than one. For metastatic disease, each drug continues for as long as it works and is tolerated.
Does trastuzumab damage the heart?
It can weaken the pumping of the heart, nearly always without symptoms and reversibly. In the HERA trial secondary heart events occurred in 4.4 percent after one year of trastuzumab. A heart scan every three months catches it early. The drug restarts once the heart recovers.
What is HER2-low?
A cancer with some HER2, scored IHC 1+ or 2+ with a negative FISH. It does not respond to trastuzumab, and in metastatic breast cancer trastuzumab deruxtecan is an option for selected patients.
Can the treatment be continued in my own country?
Yes, and for the majority it is. The first cycle is given here, and the rest by an oncologist near home on the protocol we write, with results sent back for review at no charge.
Is HER2 therapy used in stomach cancer?
Yes. One stomach cancer in five is HER2-positive, and in the ToGA trial adding trastuzumab to chemotherapy raised median survival from 11.1 to 13.8 months.
Will the staff be able to speak my language?
The international patients team works in English, Arabic, French, Russian, Serbian, Romanian and Spanish and arranges interpreters for other languages, including for the first consultation.
